Members are offered, at a substantial discount, a one-on-one Advance Care Planning session with Julianne McDevitt, an Advance Care Planning Facilitator.
A good time to solidify your plans
While members are at home practicing social distancing, learning more about advance care directives and either developing or expanding your own directives is a productive thing to do. With this in mind, the Village has arranged a discounted opportunity for our members to have a private and confidential one-hour session with Julianne.
Topics to address
At your session you will discuss your healthcare goals and wishes. If you haven’t already, you will learn how to appoint a health care agent. You can clarify questions you may have about the process of reviewing or completing your advance directive documents, such as your living will, MA health care proxy form, and more.
Your health care agent can attend
You are welcome to have your health care agent join you for this session. By having your health care agent present, this conversation will help your agent understand your goals and values, ultimately helping them make decisions for you if needed.
Fee of $25

About your facilitator
Julianne McDevitt is a registered nurse with over thirty years experience in long term care, short term rehab, hospice, emergency room and inpatient hospital case management. Julianne is a graduate of Northern Essex Community College with an Associate’s Degree in Science: RN Program. She is a graduate of the University of Vermont’s End of Life Doula program and a Respecting Choices Advance Care Planning Facilitator. Her passion is to encourage contemplation of mortality as a path to more intentional living. She resides in Salisbury, MA.
